How do I figure out the status of my request?
For status update go to app.chartrequest.com/status, or log into your account and view the request page.
If you would like to directly communicate with the Provider Organization or ChartRequest Staff, please login into the ChartRequest Platform and select the request you are inquiring about. On the bottom right hand of the screen you will see the "Provider Chat" where you can communicate with us securely and directly. We will get back to you via Provider Chat if the request has been outstanding for more than fifteen days. We aim to fulfill all requests within seven to ten business days, but some requests require access to paper records or records that have been archived and can take more time to research and securely release to you.
You will get an email notifying you when the records are available with a link to access the records!
